SECONDARY CLAIM FILING

POLICY: All CNC providers are required to submit secondary claims through CNC if the secondary coverage is provided by a CNC Contracted Payor.

At this time, secondary claims must be sent to CNC via the CMS 1500 claim form and cannot be submitted via ChiroTrack. All secondary claims and claims with any attachments must be mailed to CNC.

Please ignore the address on the member's ID card as well as any instructions you may receive when verifying benefits regarding where to submit claims. CNC instructions supersede all other instructions regarding where to submit claims.

Please note, you must have an EOB from the primary carrier before you can file the secondary claim to CNC. When you file a secondary claim to CNC, please follow these guidelines:

 

1. Generate a new CMS 1500 claim form for the same date of service including all services that were shown on the original primary claim.

a. Complete box 11d

b. Complete boxes 9a - 9d

Please note boxes 11d and 9a-9d must be completed on both the Primary and Secondary claim forms. For the Primary claim, put the primary insurance information in boxes 1a, 11 - 11c and the secondary information in boxes 9a-9d. For the Secondary claim, put the secondary insurance information in boxes 1a,

11 - 11c and the primary information in boxes 9a-9d.

2. Staple the primary EOB to the new CMS form, and include all remark codes with their explanation of the remark codes with the EOB and the secondary CMS form.

3. Mail the claim with the attached EOB to CNC at the following address. (You may mail multiple claims with their attachments all together as long as the primary EOB is STAPLED to the secondary claim.)

If you are a North Carolina provider filing BCBS as secondary coverage to Medicare, you must generate a new CMS 1500 claim form with the attached Medicare EOB and mail it to CNC. (Medicare crossover claims to BCBSNC will NOT result in payment of your claim! The claim must be filed through CNC.)
